CDI query

A CDI (clinical documentation improvement) query is a formal, compliant communication from a CDI specialist or medical coder to a physician or other clinical provider requesting clarification or additional documentation about a patient’s condition, diagnosis, or treatment. Queries are initiated when existing documentation is ambiguous, incomplete, or insufficient to support the level of coding required for accurate reimbursement, and query management is a core function of CDI programs.
